A licensed prescriber who knows the medication and the person must decide whether tapering is appropriate, how slowly to proceed, and when to pause or revise a plan.

The distinction between milligrams and percentages matters most near the lower end of a taper. Removing 5 mg from 40 mg is a 12.5% reduction, but removing 5 mg from 10 mg is a 50% reduction. A schedule can therefore look smooth on a milligram chart while becoming progressively more abrupt as a fraction of the remaining dose.

The step-count and dose formulas

Let S be the starting dose in milligrams, F the goal dose, W the duration in weeks, D the interval in days, and N the number of whole dose-change intervals. The calculator counts only complete intervals that fit into the selected window:

N = 7 × W D

A linear taper divides the total milligram difference equally among those steps:

Dose change per step = S F N

A proportional taper instead uses a constant multiplier. For a nonzero goal dose, the multiplier is the N-th root of the goal-to-start ratio:

r = ( F S ) 1 / N

The dose at step k is S multiplied by r to the power k. Because repeated multiplication by a positive ratio never equals zero, a proportional plan with a zero goal needs a separately chosen final jump. For illustration, this calculator approaches 1% of the starting dose before displaying zero as a distinct final step. That convention is not a recommendation about when to stop.

Why low-dose changes can look different

Research using positron emission tomography suggests that serotonin-transporter occupancy for several SSRIs follows a saturating rather than linear relationship with dose. One simplified teaching model is:

Worked example: 40 mg to 10 mg over 12 weeks

Suppose a person and prescriber are discussing a reduction from 40 mg to 10 mg over 12 weeks, with changes every 7 days. The 84-day window contains 12 complete weekly intervals. A linear schedule divides the 30 mg difference by 12, producing a reduction of 2.5 mg per step.

The linear sequence begins 40, 37.5, 35, and 32.5 mg before eventually reaching 10 mg. Although every cut is 2.5 mg, the first cut is 6.25% of 40 mg and the final cut is 20% of 12.5 mg. The proportional alternative uses a multiplier of approximately 0.8909, or a reduction of about 10.91% at each step. Its milligram cuts start larger and become smaller as the dose falls.

Neither sequence accounts for whether those exact doses can be dispensed. Some tablets may be safely split, some must remain intact, and some schedules require a manufacturer-supplied liquid or a compounded preparation. Interpreting and monitoring an antidepressant taper

A generated row means only that the arithmetic places a dose change on that day. It does not mean symptoms will appear immediately or resolve by the next row. During a clinician-supervised taper, it can be useful to record mood, anxiety, sleep, dizziness, nausea, sensory disturbances, concentration, energy, and any return of the condition being treated.

If symptoms become troublesome, a prescriber may recommend holding the current dose longer, returning to a previously tolerated dose, making smaller changes, or revising the overall objective. Withdrawal symptoms and recurrence of depression or anxiety can overlap, which is another reason not to diagnose the cause from a calculator schedule alone.

The calculation assumes equal time between dose changes and perfect mathematical doses.
